How to Obtain your Official Oregon Notary Seal:

• CLICK HERE to download and print the Notary Order Form.
• Fill out the order form and select your form of payment.
• Enclose the order form and your ORIGINAL"Certificate of Authorization to Obtain Official Notary Seal" in an envelope. 
• Mail your order to: ATTN: Notary Department, 335 S.E. 99th Avenue, Portland, OR 97216
• Your certificate will be returned to you with your order.  Simply mail your completed certificate to the state within 10 days of receipt.
